The objective of this study was to evaluate the efficacy of inactivated vaccines prepared with North American swine influenza viruses as well as an experimental homologous A/H1N1 vaccine to prevent infection and disease from 2009 pandemic A/H1N1. All vaccines tested provided partial protection ranging from reduction of pneumonia lesions to significant reduction in virus replication in the lung and nose. The multivalent vaccines demonstrated partial protection; however, none was able to prevent all nasal shedding or clinical disease. An experimental homologous 2009 A/H1N1 monovalent vaccine provided optimal protection with no virus detected from nose or lung at any time point in addition to amelioration of clinical disease. Conclusions The pulmonary pathologic findings in fatal disease caused by the novel pandemic influenza virus are similar to findings identified in past pandemics. Superimposed bacterial infections of the respiratory tract were common. Preexisting obesity, cardiorespiratory diseases, and other comorbidities also were prominent findings among the decedents.

To the Editor: In the late 1990s, triple-reassortant influenza A viruses containing genes from avian, human, and swine influenza viruses emerged and became enzootic in swine herds in North America (1). The first 11 human cases of novel influenza A virus infection were reported to the Centers for Disease Control and Prevention (CDC; Atlanta, GA, USA) from December 2005 through February 2009 (1). In response to those reports, surveillance for human infection with nonsubtypeable influenza A viruses was implemented.
